Insurance Coverage

Insurance

Valley Skin Institute accepts most PPO and HMO plans. For commercial HMO plans and Medicare Advantage plans, we are contracted through Saint Agnes Medical Group IPA and Santé IPA. For these plans, if your medical insurance card lists either of these IPAs, then we are in-network for your insurance.

We are now in network with Meritage Medical Group

We accept:

  • Aetna
  • Anthem Blue Cross
  • Blue Shield
  • Cigna
  • First Health/Coventry
  • Health Net
  • Health Smart
  • Kaiser (for Mohs Surgery)
  • Medicare
  • All Medicare Supplemental plans (example: AARP, Blue Cross, etc.)
  • Multiplan
  • Tricare
  • Triwest Veterans Choice
  • United Healthcare

 

Due to IPA contracting alignments, we do not accept the following insurance plans:

  • EHS (Including Medicare plans)
  • ACA (Affordable Care Act plans also known as Covered California)
  • Medi-Cal (including CalViva, EHS, LaSalle)
 
We do not accept Christian Health Sharing Programs.

Billing from Our Office

If you have questions about a bill from Valley Skin Institute, contact Hudson Medical Services at 541-322-1794.

Your Insurance Company

We will bill your insurance company directly.

Co-pays

Co-pays are due at the time of your office visit. If you have questions regarding your co-payments or deductibles, please call your insurance company.

We DO NOT accept cash at our office, so please plan accordingly.

Billing from the Lab

If you have tissue removed and sent to an off-site lab, you will receive a separate bill for those services. We typically use:

  • Golden State Dermatopathology
  • UCSF
  • Pathology Associates
